Registration Packet Forms

The following three forms must be completed for all students:

• Student Information Form
• Authorization for Medical Care and Treatment (PDF)
• Pest Control

If your student will need to take medication(s) when they are on campus, the following form must be completed by a physician:

• Authorization to Administer Medication

If your student has prescribed dietary needs, the following form must be completed by a physician:

• Request for Special Diet (PDF)

Sports Registration Forms

If your student would like to participate as part of the sports teams this year, the following form must be completed (physician signature required):
